In order for LGA to cover part of the cost for an assessment, an Authority form must be provided to CHG.
1) Navigate to the patient upload page:
a) Immediately after making a booking request, click the "View Booking" button and then click the patient.

b) Click the "My People" tab. Search or navigate to the Candidate
2) Upload the document
a) Select "Employer Authority Form" from the Document Type
b) Drag and Drop the file onto the box or click the blue box.
c) Press the Upload button
The file should appear in the Documents list at the bottom.
